Job summary

Adaptive Home Health seeks an in-home Occupational Therapist to deliver skilled OT visits and evaluations in patients' homes. You will plan care for ADLs/IADLs, safety, and independence, and coordinate with nursing for changes in condition.

Requirements

Must-haves

  • Active Texas OT license (in good standing)

  • Strong clinical judgment and comfort working independently in the field

  • Comfort escalating changes in condition to RN/MD and documenting rationale clearly

  • Strong communication and documentation

  • Reliable follow-through on orders, frequencies, and patient goals

  • Current driver's license and comfort with daily travel across assigned territory

  • Able to carry/transport basic therapy equipment as needed

  • Competency in core home health OT scope (ADL/IADL training, home safety, adaptive equipment, caregiver education)

Nice-to-haves

  • Prior home health experience

  • Deeper experience in high-signal OT populations (neuro, CVA, fall risk, dementia/caregiver training)

  • Strong organization and time management

  • Experience using charting systems / EMRs
