Job summary

Adaptive Home Health, LLC is seeking a field-based Occupational Therapist to deliver in-home skilled OT visits in Texas. You will evaluate patients, develop plans of care focused on ADLs/IADLs, and coordinate with nursing and the care team to keep patients safe and independent at home.

Responsibilities include recommending adaptive equipment, documenting thoroughly, and driving daily travel across the assigned territory. Full-time Monday-Friday schedule with field-based work model.

Qualifications

• Active Texas OT license in good standing required.
• Strong clinical judgment and comfort working independently in the field.
• Comfort escalating changes in condition to RN/MD with clear rationale.
• Strong communication and thorough documentation.
• Reliable follow-through on orders, frequencies, and patient goals.
• Current driver’s license with ability to travel daily across territory.
• Ability to transport basic therapy equipment.

Requirements

Must-haves

• Active Texas OT license (in good standing)

• Strong clinical judgment and comfort working independently in the field

• Comfort escalating changes in condition to RN/MD and documenting rationale clearly

• Strong communication and documentation

• Reliable follow-through on orders, frequencies, and patient goals

• Current driver's license and comfort with daily travel across assigned territory

• Able to carry/transport basic therapy equipment as needed

• Competency in core home health OT scope (ADL/IADL training, home safety, adaptive equipment, caregiver education)

Nice-to-haves

• Prior home health experience

• Deeper experience in high-signal OT populations (neuro, CVA, fall risk, dementia/caregiver training)

• Strong organization and time management

• Experience using charting systems / EMRs
