Job summary

Chase Center Rehabilitation and Nursing Services in Logansport, IN is seeking a motivated Occupational Therapist (OT) to join our rehab team on a PRN basis. You will assess residents, create personalized treatment plans, and support daily living activities to improve independence and safety.

Qualifications

• Licensed OT with active credential (OTR/L) in PA.
