Job summary

AlphaCare Home Health Corp. is seeking a skilled Occupational Therapist to join our team for in-home visits across Alameda County and surrounding East Bay communities.

This per diem role offers flexible scheduling without a fixed weekly timetable and focuses on patient independence, safety, and dignity at home. You will collaborate with physicians, nurses, and the broader care team to deliver thoughtful, high-quality care while building a caseload that fits your availability.

Qualifications

• Current California Occupational Therapist (OT) license required.
• Strong clinical assessment and communication skills.
• Excellent organizational and documentation abilities.
• Ability to work collaboratively with patients, families, physicians, and care teams.
• Valid California driver’s license.

Responsibilities

• Conduct initial home health OT evaluations and ongoing patient reassessments.
• Develop and implement individualized treatment plans based on physician orders.
• Monitor, assess, and document progress toward functional goals.
• Educate patients and caregivers on adaptive techniques, home safety, and equipment use.
• Coordinate care and communicate with physicians, RNs, and the clinical team.
• Communicate changes in patient condition proactively with clinical leadership.
• Participate in case conferences and care planning discussions as needed.
• Maintain timely, accurate, and compliant clinical documentation.
• Support safe discharge planning and continuity of care.
• Deliver care aligned with AlphaCare’s commitment to patient time and multilingual community care.

About the Role

We are seeking a skilled and compassionate Occupational Therapist (OT) to join our team on a per diem basis, covering patients throughout Alameda County and the surrounding East Bay.

The ideal candidate is clinically strong, patient-centered, and committed to delivering thoughtful, responsive care in the home setting. As an AlphaCare OT, you will help patients regain independence, safety, and dignity at home, coordinating care with physicians, nurses, and the broader care team.

This role is a great fit for a clinician who wants flexible, high-paying per diem work without a fixed weekly schedule — whether you're building a caseload around other commitments or simply prefer to choose your own hours.

Pay Range

$70.00 – $85.00+ per hour
