Job summary

21st Century Home Health Services is seeking a full-time Occupational Therapist to visit patients in Alameda County, Oakland, and surrounding areas. You will evaluate needs, address home safety, wheelchair seating, pain management, falls prevention, and chronic disease management while collaborating with an interdisciplinary team.

The role requires an unrestricted CA OT license, BLS/CPR, and a CA driver’s license. Knowledge of local resources and prior home health experience are highly valued.

Full Time Occupational Therapist

Territory: Alameda County: Oakland: Lake Merritt, Piedmont

What you’ll do as an Occupational Therapist (OT) Home Health:

In partnership with the Case Manager you will provide skilled services to evaluate and address the needs of individuals in their homes, so they can live safely and productively in their home environment. These areas may include home safety medication management, wheelchair seating and positioning, pain management, falls prevention, management of chronic diseases and dementia family caregiver training. You will serve as an educator, patient advocate and a member of the interdisciplinary care team

To be a success as an Occupational Therapist (OT) Home Health:

Wage Range: Occupational Therapist:$130,000 - $160,000 annually. Starting pay is commensurate with relevant experience above the minimum requirements.

The compensation structure for this position includes an annual base salary plus eligibility for an annual performance-based bonus incentive. In addition, 21st Century offers comprehensive health benefits and a 401(k) plan with up to a 4% company match.
