Job summary

GetHired, Inc. is seeking an Occupational Therapist to serve PACE participants in a community-based, long-term care model.

You will conduct evaluations and ongoing treatment focusing on ADLs, fall prevention, home safety, and caregiver education, collaborating with an interdisciplinary team. This role emphasizes geriatric care, relationship-building, and patient-centered planning in home and community settings.

Qualifications

• Master's or Doctoral degree in Occupational Therapy.
• Current or eligible for Pennsylvania State OT license.
• NBCOT certification.

Responsibilities

• Provide OT evaluations and ongoing treatment for PACE participants with focus on ADL training, fall prevention, home safety assessments, cognitive maintenance, adaptive equipment recommendations, and caregiver education.
• Collaborate with an interdisciplinary team in a community-based model that prioritizes holistic, person-centered care.
