Job summary

Evolve Therapy Services is seeking an Occupational Therapist for an in-house Skilled Nursing Facility in Beloit, Wisconsin. The role involves planning, organizing, developing, and directing occupational therapy services to ensure high-quality resident care.

Responsibilities include screening residents, evaluating and creating treatment plans, coordinating with the interdisciplinary team, documenting care, and supervising COTA as needed to maintain clinical excellence and proper reimbursement.

Qualifications

• Degree in Occupational Therapy.
• Active Occupational Therapy license in practicing state(s).
• Ongoing continuing education and professional development to maintain licensure.

Responsibilities

• Screen residents to determine best intervention and treatment.
• Evaluate residents and develop short- and long-term goals.
• Interpret and communicate evaluation findings and plans to residents and families.
• Schedule resident therapy sessions with the team.
• Document and bill OT services per plan.
• Perform duties to effectively treat patients.
• Supervise COTA and ensure proper documentation and reimbursement.

Job Position: Occupational Therapist for an In-house Skilled Nursing Facility

Job Type: Full Time

Job Purpose: To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed

Education & Qualifications:

• Degree in Occupational Therapy
• MUST have an Active Occupational Therapy license in practicing state(s)
• Ongoing continuing education and professional development to maintain current licensure and certification
