Job summary

Evolve Therapy Services is seeking a dedicated Occupational Therapist for our in-house Skilled Nursing Facility in Beloit, WI. This full-time role requires planning, organizing, developing, and directing OT services in accordance with applicable standards and regulations to ensure the highest quality resident care.

Responsibilities include screening residents, evaluating and planning treatment, coordinating schedules, and ensuring proper documentation and billing.

Qualifications

• Active Occupational Therapy license in practicing state(s).
• Ongoing continuing education to maintain licensure.

Responsibilities

• Evaluate residents to inform and develop treatment plans; set short- and long-term goals.
• Interpret and communicate findings and plans to residents, families, and interdisciplinary team.
• Establish individual resident therapy schedules with team.
• Document and bill for occupational therapy services.
• Oversee COTA and ensure documentation and treatments are carried out with quality and proper reimbursement.

Job Position: Occupational Therapist for an In-house Skilled Nursing Facility

Job Type: Full Time

Job Purpose: To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed

Education & Qualifications:

• Degree in Occupational Therapy
• MUST have an Active Occupational Therapy license in practicing state(s)
• Ongoing continuing education and professional development to maintain current licensure and certification
