Evolve Therapy Services, LLC seeks a licensed Occupational Therapist for an in-house Skilled Nursing Facility in Minneapolis, MN. The OT will plan, organize, and direct therapy services to ensure高 quality resident care in accordance with standards and regulations.

Responsibilities include evaluating residents, developing treatment plans with goals, documenting and billing, and supervising COTA as needed. Must hold OT degree and an active license, with ongoing continuing education required.

Qualifikationen

• Degree in Occupational Therapy.
• Must have an Active Occupational Therapy license in practicing state(s).
• Ongoing continuing education and professional development to maintain licensure and certification

• Screen residents to determine best intervention or treatment.
• Evaluate residents and develop treatment plans with short- and long-term goals.
• Interpret and communicate evaluation findings and recommendations to residents, families/caregivers and interdisciplinary team.
• Establish individual resident therapy schedules in collaboration with team.
• Conduct therapy according to treatment plan and document/bill services.
• Oversee COTA and ensure documentation and reimbursement practices are followed.
• Perform additional duties as assigned to effectively treat patients.

Job Position:

Occupational Therapist for an In-house Skilled Nursing Facility

Job Type:

Full Time

Job Purpose:

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient


• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals


• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members


• Establish individual resident therapy schedules in cooperation with other team members


• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services


• Perform all other duties as assigned and as required to effectively treat patients


• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed

Education & Qualifications:

• Degree in Occupational Therapy


• MUST have an Active Occupational Therapy license in practicing state(s)


• Ongoing continuing education and professional development to maintain current licensure and certification
