Evolve Therapy Services is hiring an Occupational Therapist for an in-house Skilled Nursing Facility located in Rochester, MN. The role supports full time, part time, or PRN schedules to meet residents' needs.

Responsibilities include evaluating residents, developing treatment plans, scheduling therapy, communicating with families, and ensuring accurate documentation and billing. Licensure is required and ongoing CEUs are encouraged to maintain credentials.

Qualifications

• Degree in Occupational Therapy required.
• Active Occupational Therapy license in practicing state(s) required.
• Ongoing continuing education to maintain licensure and certification required.

Responsibilities

• Screen residents to determine best intervention or treatment.
• Evaluate residents and develop treatment plans with short- and long-term goals.
• Interpret and communicate evaluation findings and plans to residents, families, and interdisciplinary team.
• Establish individual resident therapy schedules with team.
• Conduct therapy per plan and ensure documentation and billing accuracy.
• Perform all duties to effectively treat patients and ensure clinical excellence and proper reimbursement.
• Oversee COTA as needed and ensure documentation and treatments are carried out correctly.

Job Position: Occupational Therapist for an In-house Skilled Nursing Facility

Job Type: Full Time, Part Time, or PRN positions available

Job Purpose: To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed

Education & Qualifications:

• Degree in Occupational Therapy
• MUST have an Active Occupational Therapy license in practicing state(s)
• Ongoing continuing education and professional development to maintain current licensure and certification
