Job summary

Evolve Therapy Services is hiring an Occupational Therapist for an in-house Skilled Nursing Facility located in MN. This PRN, telehealth-remote role requires delivering occupational therapy services to residents in coordination with a facility-based team.

You will screen residents, develop treatment plans, communicate findings to families and the interdisciplinary team, and oversee COTA as needed while ensuring proper documentation and billing, all while maintaining clinical excellence.

Qualifications

• Must hold an active OT license in the practicing state(s).
• Degree in Occupational Therapy.
• Ongoing continuing education to maintain licensure.

Responsibilities

• Screen residents to determine best interventions and treatment.
• Evaluate residents and develop short- and long-term goals.
• Interpret and communicate evaluation findings to residents, families, and team.
• Establish resident therapy schedules in collaboration with the team.
• Document and bill OT services per protocols and plans.
• Perform duties to effectively treat patients.
• Oversee COTA as needed and ensure proper documentation and reimbursement.

Job Position: Occupational Therapist for an In-house Skilled Nursing Facility

Job Type: PRN (Telehealth - Remote)

Job Purpose: To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient

• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals

• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members

• Establish individual resident therapy schedules in cooperation with other team members

• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services

• Perform all other duties as assigned and as required to effectively treat patients

• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed

Education & Qualifications:

• Degree in Occupational Therapy

• MUST have an Active Occupational Therapy license in practicing state(s)

• Ongoing continuing education and professional development to maintain current licensure and certification
