Job summary

Evolve Therapy Services is seeking a dedicated Occupational Therapist for an in-house Skilled Nursing Facility partner in Clarion, PA. This full-time or PRN position focuses on delivering comprehensive OT services to residents.

You will screen residents, develop and monitor treatment plans, document care, and supervise COTA staff, ensuring regulatory compliance and high-quality patient outcomes.

Qualifications

• Degree in Occupational Therapy.
• Active OT license in practicing state(s).
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Screen residents to identify best intervention strategies.
• Evaluate residents and develop treatment plans with goals.
• Communicate findings to residents, families, and care team.
• Establish resident therapy schedules with the team.
• Deliver therapy per plan and handle documentation and billing.
• Oversee COTA and ensure clinical excellence and proper reimbursement.
• Perform other duties as needed to effectively treat patients.

Job Position: Occupational Therapist for an In-house Skilled Nursing Facility

Job Type: Full Time or PRN

Job Purpose: To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient

• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals

• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members

• Establish individual resident therapy schedules in cooperation with other team members

• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services

• Perform all other duties as assigned and as required to effectively treat patients

• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed

Education & Qualifications:

• Degree in Occupational Therapy

• MUST have an Active Occupational Therapy license in practicing state(s)

• Ongoing continuing education and professional development to maintain current licensure and certification
