Job summary

Evolve Therapy Services is seeking a licensed Occupational Therapist for an in-house Skilled Nursing Facility in Clarion, PA. You will plan, organize, and direct OT services to ensure high-quality resident care in accordance with regulations.

Responsibilities include screening residents, developing treatment plans, documenting and billing, supervising COTA, and ensuring compliance with guidelines. Active OT license is required; competitive pay and growth opportunities are available.

Qualifications

• Degree in Occupational Therapy required.
• Active Occupational Therapy license in practicing state(s) required.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Screen residents to determine best intervention for each patient.
• Evaluate residents to inform treatment plans and establish goals.
• Interpret and communicate evaluation findings to residents, families and the interdisciplinary team.
• Establish resident therapy schedules with the team.
• Conduct therapy per treatment plan and document/bill appropriately.
• Perform all other duties as assigned to treat patients.
• Oversee COTA and ensure documentation and reimbursement practices are followed.

Job Position: Occupational Therapist for an In-house Skilled Nursing Facility

Job Type: Full Time or PRN

Job Purpose: To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient

• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals

• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members

• Establish individual resident therapy schedules in cooperation with other team members

• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services

• Perform all other duties as assigned and as required to effectively treat patients

• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed

Education & Qualifications:

• Degree in Occupational Therapy

• MUST have an Active Occupational Therapy license in practicing state(s)

• Ongoing continuing education and professional development to maintain current licensure and certification
