Evolve Therapy Services is seeking a compassionate Occupational Therapist to join our in-house Skilled Nursing Facility in Kenosha, Wisconsin. This PRN position offers flexible hours with opportunities for consistent or casual scheduling, allowing you to deliver high-quality therapy and contribute to resident well-being.

You will plan, implement, and document OT services, collaborate with a multidisciplinary team, oversee COTA when applicable, and maintain licensure with ongoing education.

Qualificações

• Must have a Degree in Occupational Therapy.
• Must have an Active Occupational Therapy license in practicing state(s).
• Ongoing continuing education and professional development to maintain licensure and certification

Responsabilidades

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient.
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals.
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members.
• Establish individual resident therapy schedules in cooperation with other team members.
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services.
• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed.
• Perform all other duties as assigned and as required to effectively treat patients

Job Position: Occupational Therapist for an In-house Skilled Nursing Facility

Job Type: PRN (Consistent or Casual Hours available)

Job Purpose: To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.
