Job summary

Evolve Therapy Services in Bradford, PA is seeking a licensed Occupational Therapist for an in-house Skilled Nursing Facility. This PRN position focuses on delivering comprehensive OT services in collaboration with an interdisciplinary team.

You will screen residents, develop goals, implement treatment plans, document progress and coordinate with COTA under supervision, ensuring regulatory compliance and quality care.

Qualifications

• Must have an active Occupational Therapy license in the practicing state(s).
• Degree in Occupational Therapy is required.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Conduct regular screenings of residents to determine the best intervention or treatment for each patient.
• Evaluate residents to inform and develop treatment plans; set short- and long-term goals and methods to achieve them.
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members.
• Establish individual resident therapy schedules in cooperation with other team members.
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services.
• Perform all other duties as assigned to effectively treat patients.
• Oversee COTA as needed and ensure documentation and treatments are carried out with clinical excellence and proper reimbursement practices.

Job Purpose:

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:
