Job summary

Evolve Therapy Services, LLC seeks an Occupational Therapist to join an in-house Skilled Nursing Facility team in Warren, PA. This PRN role emphasizes planning, delivering, and coordinating occupational therapy services with the interdisciplinary team, while ensuring accurate documentation and billing.

The candidate must hold an active OT license for the practicing state, and demonstrate a commitment to high-quality resident care within applicable standards and guidelines.

Qualifications

• Degree in Occupational Therapy required.
• Active Occupational Therapy license in practicing state(s) required.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Conduct regular assessment and evaluation of residents to inform treatment plans.
• Develop and implement individualized OT treatment plans with measurable goals.
• Communicate evaluation findings and recommendations to residents, families, and the interdisciplinary team.
• Coordinate therapy schedules with team members and ensure proper documentation and billing.

Job Position: Occupational Therapist for an In-house Skilled Nursing Facility
Job Type: PRN
Job Purpose: To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.
Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed
  

Education & Qualifications:

• Degree in Occupational Therapy
• MUST have an Active Occupational Therapy license in practicing state(s)
• Ongoing continuing education and professional development to maintain current licensure and certification
