Job summary

Evolve Therapy Services, LLC seeks a motivated Occupational Therapist for an in-house Skilled Nursing Facility in East Moline, IL. The OT will plan, direct, and oversee therapy services to maintain high-quality resident care.

The role requires an active OT license in Illinois, a degree in Occupational Therapy, and ongoing continuing education. Full-time or PRN positions with opportunities for growth are available, with competitive pay and benefits based on status.

Qualifications

• Degree in Occupational Therapy required.
• Active OT license in practicing state(s).
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Screen residents to determine the best intervention plan.
• Evaluate residents and develop treatment plans with short- and long-term goals.
• Interpret and communicate evaluation findings to residents, families/caregivers, and interdisciplinary team members.
• Establish individual resident therapy schedules with other team members.
• Conduct therapy according to the treatment plan and document and bill OT services accurately.
• Perform other duties as assigned to effectively treat patients.
• Oversee COTA as needed and ensure proper documentation and reimbursement practices.

Job Position:

Occupational Therapist for an In-house Skilled Nursing Facility

Job Type:

Full Time or PRN

Job Purpose:

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed

Education & Qualifications:

• Degree in Occupational Therapy
• MUST have an Active Occupational Therapy license in practicing state(s)
• Ongoing continuing education and professional development to maintain current licensure and certification
