Job summary

Evolve Therapy Services, LLC in Fremont and Tiffin, OH seeks an Occupational Therapist for an in-house Skilled Nursing Facility to plan, direct and oversee therapy services.

You will evaluate residents, set goals, communicate findings to families and the care team, schedule therapy, document care and support proper reimbursement, while guiding COTAs as needed.

Qualifications

• Degree in Occupational Therapy.
• MUST have an Active Occupational Therapy license in practicing state(s).
• Ongoing continuing education and professional development to maintain current licensure and certification.

Responsibilities

• Conduct regular screening of residents to identify best interventions.
• Evaluate residents to inform and develop treatment plans; set short- and long-term goals.
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members.
• Establish individual resident therapy schedules in cooperation with other team members.
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services.
• Perform all other duties as assigned and as required to effectively treat patients.
• Oversee COTA as needed and ensure documentation and treatments are carried out with clinical excellence and proper reimbursement practices.

Job Purpose

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions
