Job summary

Evolve Therapy Services, LLC in Omaha, NE, seeks a licensed Occupational Therapist for an in-house Skilled Nursing Facility position. You will plan and direct OT services, collaborate with the care team, and ensure compliance with regulations while delivering high quality resident care.

The role emphasizes ongoing professional development, patient-centered treatment planning, and effective documentation to support reimbursement and quality outcomes.

Qualifications

• Degree in Occupational Therapy.
• Active Occupational Therapy license in practicing state(s).
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Conduct regular screening of residents to determine the best intervention or treatment for each patient.
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals.
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members.
• Establish individual resident therapy schedules in cooperation with other team members.
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services.
• Perform all other duties as assigned and as required to effectively treat patients.
• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed.

Job Purpose

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.
