Job summary

Evolve Therapy Services is seeking a dedicated Occupational Therapist for an in-house Skilled Nursing Facility in Lincoln, Nebraska. This full-time position requires planning, directing, and delivering comprehensive OT services in accordance with federal, state, and local standards, under the direction of the Administrator.

You will screen residents, develop treatment plans, and coordinate care with an interdisciplinary team to ensure high-quality resident outcomes.

Qualifications

• Degree in Occupational Therapy is required.
• Active OT license in practicing states is required.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Conduct regular screenings of residents to identify interventions.
• Evaluate residents to develop treatment plans and set short/long-term goals.
• Interpret and communicate evaluation findings to residents, families, and team members.
• Establish individual resident therapy schedules with team members.
• Conduct therapy per treatment plan and document/bill services accurately.
• Perform all duties to effectively treat patients within the facility.
• Oversee COTA and ensure compliant documentation and reimbursement practices.

Job Position: Occupational Therapist for an In-house Skilled Nursing Facility

Job Type: Full Time

Job Purpose: To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed

Education & Qualifications:

• Degree in Occupational Therapy
• MUST have an Active Occupational Therapy license in practicing state(s)
• Ongoing continuing education and professional development to maintain current licensure and certification
