Job summary

Evolve Therapy Services, LLC is seeking an Occupational Therapist for an in-house Skilled Nursing Facility in Sutherland, NE. This PRN opportunity requires an active OT license in the state and involves planning, directing, and documenting occupational therapy services to maintain high-quality resident care.

You will conduct screenings, develop and adjust treatment plans, communicate with residents and families, and coordinate therapy schedules with the interdisciplinary team.

Qualifications

• Plan and direct Occupational Therapy services in the facility.
• Screen residents to determine best interventions.
• Evaluate residents and establish goals and treatment plans.
• Communicate findings to residents, families, and the interdisciplinary team.
• Coordinate therapy schedules with other team members.
• Perform documentation and billing for OT services.
• Supervise COTA as needed and ensure compliance with reimbursement practices.

Responsibilities

• Interpret and communicate evaluation findings to residents, families, and the interdisciplinary team.

Job Position

Occupational Therapist for an In-house Skilled Nursing Facility

Job Purpose

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed

Education & Qualifications

• Degree in Occupational Therapy
• MUST have an Active Occupational Therapy license in practicing state(s)
• Ongoing continuing education and professional development to maintain current licensure and certification
