Job summary

Evolve Therapy Services is seeking a dedicated Occupational Therapist to lead in-house therapy services at our partner facility in Omaha, Nebraska. The role focuses on planning, organizing, and directing OT services to ensure high-quality resident care in accordance with laws and regulations.

You will screen residents, evaluate needs, develop treatment plans with clear goals, coordinate with families and the care team, and supervise COTAs while ensuring proper documentation and billing.

Qualifications

• Degree in Occupational Therapy.
• Active OT license in practicing state(s).
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Screen residents to determine the best intervention or treatment for each patient.
• Evaluate residents to inform and develop treatment plans; establish goals and methods to achieve them.
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and the interdisciplinary team.
• Establish individual resident therapy schedules in cooperation with other team members.
• Conduct therapy according to the established plan and ensure proper documentation and billing of services.
• Perform all other duties as assigned and as required to effectively treat patients.
• Oversee COTA as needed and ensure documentation and treatments are carried out with clinical excellence and proper reimbursement practices.

Job Position:

Occupational Therapist for an In-house Skilled Nursing Facility

Job Type:

Full Time

Job Purpose:

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members

• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients

Education & Qualifications:
