Occupational Therapist (OT) - BAYADA Home Health Care

Position: Full-time or part-time Occupational Therapist (OT)

Location: Ohio Visits office, servicing Southern Cuyahoga County and Northern Summit County

Overview: Join BAYADA as an essential member of a multidisciplinary team providing rehabilitative and skilled nursing care to adult and geriatric patients in their homes, enabling them to remain independent.

Responsibilities:

• Make home visits to clients within designated geographic areas.
• Perform comprehensive evaluations of patients’ functional status, including muscle function, endurance, visual coordination, communication skills, and ADL/IADL performance.
• Assess cognitive, psychological, and motor performance; develop individualized intervention plans.
• Evaluate the home environment for hazards and barriers to independence.
• Identify equipment needs and environmental modifications to improve outcomes.
• Continuously assess and update the care plan; collaborate with nursing staff as part of the multidisciplinary team.
• Educate patients and families on rehabilitative activities to promote health, safety, and independence.
• Document observations, interventions, and evaluations accurately using a touch pad tablet.

Qualifications:

• Current Ohio Occupational Therapist license.
• Minimum of one year recent experience as an OT; home care experience preferred but not required.
• Graduation from an accredited occupational therapy program.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Proficiency with computers; EMR experience preferred.
