Job description

BAYADA Home Health Care

is currently seeking an experienced Occupational Therapist, OT for a full-time opportunity performing home health visits for our Northampton-Monroe Visits office located in Allentown, PA.

This office services adult and geriatric patients on a per-visit basis and is looking for an OT to cover home visits throughout Easton, PA.

As a home care Occupational Therapist, you will be an integral member of a multi-disciplinary healthcare team that provides rehabilitative care and skilled nursing, enabling clients to receive necessary medical care to remain at home.

Occupational Therapist, OT Responsibilities:

1. Make home visits to clients in designated geographic areas.
2. Perform evaluations of patients' functional status, including muscle function, endurance, visual coordination, communication skills, and ADL/IADL performance.
3. Assess cognitive, psychological, and motor performance aspects and develop personalized intervention plans.
4. Evaluate the home environment for hazards or barriers to independence.
5. Identify equipment needs and environmental modifications to improve outcomes.
6. Continuously assess and update the occupational therapy care plan and collaborate with nursing in the multidisciplinary care planning.
7. Educate patients, families, or caregivers on rehabilitative activities to promote health, safety, and independence.
8. Document observations, interventions, and evaluations accurately using a touch tablet device.

Occupational Therapist, OT Qualifications:

1. Current PA Occupational Therapist license.
2. Prior home care experience preferred but not required.
3. At least one year of recent experience as an Occupational Therapist.
4. Graduation from an accredited occupational therapy program.
5. Ability to work independently and manage time effectively.
6. Strong interpersonal, organizational, and problem-solving skills.
7. Proficiency with computer systems; EMR experience preferred.
