Job Opportunity: Occupational Therapist (OT) at BAYADA Home Health Care

Position: Full-time or Part-time Occupational Therapist (OT)

Location: Ohio Visits Office, serving West Side of Cuyahoga County and Medina County

Overview: BAYADA Home Health Care is seeking an experienced Occupational Therapist to provide home health visits. Join a multidisciplinary team dedicated to delivering rehabilitative and skilled nursing care, enabling clients to remain safely at home.

Responsibilities:

1. Conduct home visits within designated geographic areas.
2. Perform comprehensive evaluations of patients' functional abilities, including muscle function, endurance, coordination, communication, and ADL/IADL performance.
3. Assess cognitive, psychological, and motor performance; develop individualized intervention plans.
4. Evaluate the home environment for hazards and barriers.
5. Identify equipment needs and environmental modifications to improve client outcomes.
6. Regularly update care plans and collaborate with nursing staff as part of the multidisciplinary team.
7. Educate patients and families on rehabilitative activities to promote health and independence.
8. Document patient observations, interventions, and evaluations accurately using electronic tablets.

Qualifications:

• Current Ohio Occupational Therapist license.
• Minimum one year of recent OT experience; home care experience preferred but not required.
• Graduation from an accredited occupational therapy program.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Proficiency with computers; EMR experience preferred.
