Location: Wake County, Raleigh, NC

As a home care Occupational Therapist, you will be an integral member of a multi-disciplinary healthcare team providing rehabilitative care and skilled nursing to clients, enabling them to remain at home.

Position Details:

• Full-time home health visits in Wake County, NC.
• Servicing adult and geriatric patients in their homes.

Responsibilities:

• Make home visits to clients in designated areas.
• Perform evaluations of patients’ functional status, including muscle function, endurance, visual coordination, communication skills, and ADL/IADL performance.
• Assess cognitive, psychological, and motor aspects; develop individualized intervention plans.
• Evaluate home environments for hazards or barriers.
• Identify equipment needs and environmental modifications.
• Reassess and revise care plans; collaborate with nursing staff.
• Educate patients and families on rehabilitative activities.
• Document observations and interventions accurately using electronic tablets.

Qualifications:

• Current North Carolina Occupational Therapist license.
• Minimum one year of recent work experience as an OT.
• Graduation from an accredited occupational therapy program.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.
