BAYADA Home Health Care is seeking an experienced Occupational Therapist (OT) for a full-time or per diem role providing home health visits for our Person County Visits office in Roxboro, North Carolina. This office serves adult and geriatric patients across Granville, Vance, Franklin, Person, Caswell, and Orange counties.

Role Description

As a home care Occupational Therapist, you will be an essential part of a multidisciplinary healthcare team, delivering rehabilitative care and skilled nursing to enable clients to remain at home.

Responsibilities

1. Conduct home visits within designated geographic areas.
2. Perform comprehensive evaluations of patients’ functional abilities, including muscle strength, endurance, coordination, communication skills, and ADL/IADL performance.
3. Assess cognitive, psychological, and motor performance, and develop tailored intervention plans.
4. Inspect the home environment for hazards or barriers to independence.
5. Identify necessary equipment and environmental modifications to improve outcomes.
6. Regularly review and update the occupational therapy care plan and collaborate with nursing staff in multidisciplinary planning.
7. Educate patients and their families on rehabilitative activities to promote health, safety, and independence.
8. Document all observations, interventions, and evaluations accurately using a modern touchpad tablet.

Qualifications

1. Current North Carolina Occupational Therapist license.
2. Preferably, previous home care experience, but not mandatory.
3. At least one year of recent experience as an Occupational Therapist.
4. Graduation from an accredited occupational therapy program (transcript or diploma required).
5. Ability to work independently and efficiently manage time.
6. Strong interpersonal, organizational, and problem-solving skills.
7. Proficiency in computer skills; experience with electronic medical records (EMR) preferred.
