BAYADA Home Health Care

Currently seeking an experienced Occupational Therapist (OT) for a full-time or part-time opportunity providing home health visits for our Ohio Visits office. This office services adult and geriatric patients on a per-visit basis throughout West Side of Cuyahoga County and Medina County.

As a home care Occupational Therapist, you will be a vital member of a multi-disciplinary healthcare team that offers rehabilitative care and skilled nursing, enabling clients to receive necessary medical care at home.

Occupational Therapist, OT Responsibilities:

• Make home visits to clients in designated geographic areas.
• Perform comprehensive evaluations of patients’ functional status, including muscle function, endurance, visual coordination, communication skills, and ADL/IADL performance.
• Assess cognitive, psychological, and motor performance aspects; develop individualized intervention plans for favorable outcomes.
• Evaluate home environments for hazards or barriers to independent living.
• Identify equipment needs and environmental modifications to improve client outcomes.
• Continuously assess and update the occupational therapy care plan, collaborating with nursing staff as needed.
• Educate patients, family members, or caregivers on rehabilitative care and activities to promote health, safety, and independence.
• Document observations, interventions, and evaluations accurately using a state-of-the-art touch pad tablet.

Occupational Therapist, OT Qualifications:

• Current OH Occupational Therapist license.
• Prior home care experience preferred but not required.
• Minimum of one year of recent experience as an Occupational Therapist.
• Graduation from an accredited occupational therapy program recognized by the AMA and AOTA.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Proficiency with computers; experience with electronic medical records (EMR) preferred.
