BAYADA Home Health Care is currently seeking an experienced Occupational Therapist (OT) for a per diem opportunity performing home health visits for our adult and geriatric clients.

Coverage area: Greenville, Pickens, Spartanburg, & Anderson Counties

As a home care Occupational Therapist, you will be an integral member of a multidisciplinary healthcare team that provides rehabilitative care and skilled nursing to clients, enabling them to remain at home.

Occupational Therapist, OT Responsibilities:

1. Make home visits to clients in designated geographic territories.
2. Perform evaluations of patients’ functional status, including muscle function, endurance, visual coordination, communication skills, and ADL and IADL performance.
3. Assess cognitive, psychological, and motor aspects of performance and develop individualized intervention plans.
4. Evaluate home environments for hazards or barriers to independent living.
5. Identify equipment needs and environmental adaptations to improve client outcomes.
6. Assess and revise occupational therapy care plans and collaborate with nursing in multidisciplinary care planning.
7. Educate patients, family members, or representatives in rehabilitative care and activities to promote health, safety, and independence.
8. Document observations, interventions, and evaluations accurately using a touch pad tablet.

Occupational Therapist, OT Qualifications:

1. Current SC Occupational Therapist license.
2. Prior home care experience preferred but not required.
3. At least one year of recent experience as an Occupational Therapist.
4. Graduation from an accredited occupational therapy program.
5. Ability to work independently and manage time effectively.
6. Strong interpersonal, organizational, and problem-solving skills.
7. Solid computer skills; experience with electronic medical records (EMR) preferred.
