Occupational Therapist (OT) - Home Health

Join BAYADA Home Health Care as an Occupational Therapist (OT) in our Pitt County office, located in Greenville, NC. This per diem role involves providing home health visits to adult and geriatric patients in their homes throughout Pitt County.

As a vital part of our multidisciplinary team, you will deliver rehabilitative care and skilled nursing, enabling clients to remain at home with the necessary medical support.

Responsibilities:

1. Conduct home visits to clients within designated areas.
2. Perform comprehensive evaluations of patients’ functional status, including muscle function, endurance, visual coordination, communication skills, and ADL/IADL performance.
3. Assess cognitive, psychological, and motor performance; develop individualized intervention plans.
4. Evaluate home environments for hazards or barriers to independence.
5. Identify equipment needs and environmental modifications to improve outcomes.
6. Update and revise care plans; collaborate with nursing staff in multidisciplinary planning.
7. Educate patients and families on rehabilitative activities to promote health and safety.
8. Document observations and interventions accurately using electronic tablets.

Qualifications:

1. Current North Carolina Occupational Therapist license.
2. At least one year of recent OT experience; home care experience preferred but not required.
3. Graduation from an accredited occupational therapy program.
4. Ability to work independently and manage time effectively.
5. Strong interpersonal, organizational, and problem-solving skills.
6. Proficiency with computers; EMR experience preferred.
