Job summary

Linwood Meadows Care Center in Visalia, CA is seeking a compassionate Occupational Therapist (OT) for a full-time role. You will evaluate residents, design individualized treatment plans, and provide therapeutic interventions to improve independence in daily living skills and mobility.

The position offers competitive pay at $60–$65 per hour, a per diem-friendly schedule, ongoing training, and opportunities to contribute to a resident-focused care culture with strong teamwork and support.

Qualifications

• Bachelor's Degree in Occupational Therapy required.
• Master's or Doctorate in OT preferred.
• Current California OT license in good standing.
• Experience with Casamba and PointClickCare preferred.

Responsibilities

• Evaluate residents and determine OT needs and rehabilitation potential.
• Develop, implement, and modify individualized treatment plans.
• Provide interventions focused on ADLs, strength, coordination, balance, cognition, and safety.
• Communicate with physicians, nursing staff, residents, and families on progress and goals.
• Participate in care plan meetings and interdisciplinary team conferences.
• Complete timely, accurate documentation compliant with regulations.
• Educate residents and caregivers on home programs and fall prevention.
• Recommend adaptive equipment and supervise COTAs and therapy aides as appropriate.
• Maintain a safe treatment environment and follow infection control procedures.

General Purpose

The Occupational Therapist evaluates residents, develops individualized treatment plans, and provides therapeutic interventions to improve independence with daily living skills, mobility, safety, and overall functional performance.

Essential Duties

• Evaluate residents and determine occupational therapy needs and rehabilitation potential
• Develop, implement, and modify individualized treatment plans
• Provide therapeutic interventions focused on ADLs, strength, coordination, balance, cognition, and safety
• Communicate with physicians, nursing staff, residents, and families regarding progress and goals
• Participate in care plan meetings and interdisciplinary team conferences
• Complete timely, accurate, and compliant documentation
• Educate residents and caregivers on home programs, adaptive strategies, and fall prevention
• Recommend adaptive equipment and assistive devices to promote independence
• Supervise COTAs and therapy aides as appropriate
• Maintain a safe treatment environment and follow infection control procedures

Qualifications

Education

• Bachelor's Degree in Occupational Therapy required
• Master's or Doctorate preferred

License

• Current California Occupational Therapy license in good standing
