Outcome HC seeks an Occupational Therapist (OT) to join our skilled nursing facility team in Mahoning Post Acute Nursing and Rehab. You will assess residents, create personalized treatment plans, and deliver therapies to improve independence, safety, and functional abilities.

The role emphasizes collaboration with an interdisciplinary team, documentation for billing, and adherence to care standards. A valid OT license and strong clinical reasoning are required, with geriatric experience

Responsibilities

• Provide direct therapy services including therapeutic exercises, ADL retraining, cognitive interventions, and adaptive equipment training.
• Recommend and train residents on assistive devices and adaptive equipment.
• Document evaluations, progress notes, and discharge summaries per regulatory standards.

We are seeking a dedicated and compassionate Occupational Therapist (OT) to join our interdisciplinary team in a skilled nursing facility. The ideal candidate is committed to improving the quality of life for older adults through evidence-based, person-centered care. This role involves evaluating residents, developing individualized treatment plans, and delivering therapeutic interventions that promote independence, safety, and functional ability.

Clinical Care

• Conduct comprehensive evaluations of residents’ physical, cognitive, and psycho-social abilities.

• Develop individualized treatment plans focused on functional improvement, ADLs, mobility, and safety.

• Provide direct therapy services including therapeutic exercises, ADL retraining, cognitive interventions, and adaptive equipment training.

• Recommend and train residents on the use of assistive devices and adaptive equipment.

• Document evaluations, progress notes, and discharge summaries in accordance with regulatory and facility standards.

Interdisciplinary Collaboration

• Communicate treatment goals and progress with residents, families, and caregivers.

• Provide education on safe mobility, fall prevention, and home safety strategies.

Regulatory & Administrative

• Maintain accurate and timely documentation for billing and reimbursement.

• Adhere to facility policies, professional ethics, and best-practice standards.

Qualifications
