Job summary

FeldCare Connects is seeking a self-motivated Occupational Therapist to deliver premier care to adults in the Santa Maria area as an Independent Contractor.

Your duties include assessing patient needs related to daily living, planning and delivering therapy, and coordinating with the health care team while maintaining balance and autonomy in a flexible schedule.

The role emphasizes functional assessment, home safety, adaptive equipment guidance, and ongoing progress evaluation to support recovery.

Qualifications

• Ability and enthusiasm in working with all ages.
• Licensed and registered as Occupational Therapist in the state.
• Completion of an accredited Occupational Therapist program.

Responsibilities

• Assess patient care needs related to functional status and ADLs.
• Develop and implement an OT treatment plan and interventions.
• Monitor and document patient progress and response to treatment.
• Collaborate with the health care team to coordinate care.

FeldCare Connects, an app based referral network,is currently seeking a self-motivated Occupational Therapist to deliver premier excellence of care and is enthusiastic about working with adults of all ages.

This position is for an Independent Contractor to serve the Santa Maria area specifically.

The Occupational Therapist in Home Health is responsible for the assessment and evaluation of patient care needs relating to functionality status, activities of daily living, fine motor coordination, home assessments and adaptive equipment, and other occupational therapy needs as defined by medical conditions. Based on this assessment and evaluation, the Occupational Therapist determines a treatment plan, performs interventions aimed at improving and enhancing the patient's wellbeing, and evaluates the patient’s progress.

An OccupationalTherapist for Home Health must:

• Provide services defined by the state laws governing the practice of Occupational Therapy, in accordance with the plan of care, and in coordination with other members of the health care team
• Lead the patient in the use of therapeutic, creative, and self-care activities to improve functioning and safety
• Observe, record, and report the patient's response to treatment and changes to the patient's condition
• Instruct patients, families, caregivers, and the health care team in the areas of therapy in which they can participate in assisting the patient

Qualifications:

• Ability and enthusiasm in working with all ages
• Occupational Therapistlicense and registration by the state
• Completion of an accredited Occupational Therapistprogram
