Job summary

FeldCare Connects is seeking a self-motivated Occupational Therapist to deliver premier care for adults in Moss Landing and surrounding areas. This Independent Contractor role focuses on home health assessment, treatment planning, and progress evaluation to enhance patients’ daily functioning.

The OT will assess ADLs, fine motor skills, and adaptive equipment needs, coordinating with the care team to ensure quality outcomes for clients across various ages.

Qualifications

• Must hold OT license and state registration.
• Completion of an accredited OT program.
• Willingness to work with all ages; bilingual a plus.

Responsibilities

• Assess patient care needs and functional status to determine care plan.
• Develop and implement OT treatment plans to improve wellbeing.
• Evaluate patient progress and adjust interventions accordingly.
• Conduct home assessments and recommend adaptive equipment.
• Coordinate with the health care team per the plan of care.

FeldCare Connects, an app-based referral network, is currently seeking a self-motivated Occupational Therapist to deliver premier excellence of care and is enthusiastic about working with adults of all ages.

This position is for an Independent Contractor to serve Moss Landing and the surrounding areas.

The Occupational Therapist in Home Health is responsible for the assessment and evaluation of patient care needs relating to functionality status, activities of daily living, fine motor coordination, home assessments and adaptive equipment, and other occupational therapy needs as defined by medical conditions. Based on this assessment and evaluation, the Occupational Therapist determines a treatment plan, performs interventions aimed at improving and enhancing the patient's wellbeing, and evaluates the patient’s progress.

An OccupationalTherapist for Home Health must:

• Provide services defined by the state laws governing the practice of Occupational Therapy, in accordance with the plan of care, and in coordination with other members of the health care team.
• Lead the patient in the use of therapeutic, creative, and self-care activities to improve functioning and safety.
• Observe, record, and report the patient's response to treatment and changes to the patient's condition.
• Instruct patients, families, caregivers, and the health care team in the areas of therapy in which they can participate in assisting the patient.

Qualifications:

• Ability and enthusiasm in working with all ages
• Occupational Therapistlicense and registration by the state
• Completion of an accredited Occupational Therapistprogram
• Bilingual is a plus!
