Job summary

FeldCare Connects in Santa Clarita is seeking a self-motivated Occupational Therapist to deliver premier home health care for adults of all ages. This Independent Contractor position covers Santa Clarita and surrounding areas.

You will assess functional status, ADLs, fine motor skills, and adaptive equipment, develop a treatment plan, and monitor patient progress. Applicants must hold a state-registered OT license, complete an accredited OT program, and be able to work with diverse patients;

Qualifications

• Ability and enthusiasm in working with all ages.
• Licensed and registered OT in the state.
• Completion of an accredited OT program.

Responsibilities

• Assess patients and evaluate needs related to ADLs, functional status, and home safety.
• Develop and implement OT treatment plans and interventions.
• Provide training to patients, families, and care teams.
• Monitor progress and adjust care plans as needed.

FeldCare Connects, an app-based referral network,is currently seeking a self-motivated Occupational Therapist to deliver premier excellence of care and is enthusiastic about working with adults of all ages.

This position is for an Independent Contractor to serve Santa Clarita and the surrounding areas.

Occupational Therapist in Home Health is responsible for the assessment and evaluation of patient care needs relating to functionality status, activities of daily living, fine motor coordination, home assessments and adaptive equipment, and other occupational therapy needs as defined by medical conditions. Based on this assessment and evaluation, the Occupational Therapist determines a treatment plan, performs interventions aimed at improving and enhancing the patient's wellbeing, and evaluates the patient’s progress.

An OccupationalTherapist for Home Health must:

• Provide services defined by the state laws governing the practice of Occupational Therapy, in accordance with the plan of care, and in coordination with other members of the health care team.
• Lead the patient in the use of therapeutic, creative, and self-care activities to improve functioning and safety.
• Observe, record, and report the patient's response to treatment and changes to the patient's condition.
• Instruct patients, families, caregivers, and the health care team in the areas of therapy in which they can participate in assisting the patient.

Qualifications:

• Ability and enthusiasm in working with all ages
• Occupational Therapistlicense and registration by the state
• Completion of an accredited Occupational Therapistprogram
• Bilingual is a plus!
