Job summary

FeldCare Connects seeks a self-motivated Occupational Therapist to deliver premier care in home health for adults of all ages. This Independent Contractor role serves the San Gabriel Valley area. You will assess needs, plan treatments, and support patient independence.

Responsibilities include leading patients in therapy, coordinating with the health care team, and documenting progress to ensure high-quality outcomes for diverse clients.

Qualifications

• Licensed and registered as an Occupational Therapist by the state.
• Completion of an accredited Occupational Therapist program.
• Ability and enthusiasm in working with all ages.
• Bilingual is a plus.

Responsibilities

• Assess and evaluate patient care needs related to functionality and ADLs.
• Develop a treatment plan and perform interventions to improve wellbeing.
• Evaluate patient progress and report changes to the health care team.
• Instruct patients, families, caregivers in therapy areas to participate in care.

Job description

FeldCare Connects, an app-based referral network,is currently seeking a self-motivated Occupational Therapist to deliver premier excellence of care and is enthusiastic about working with adults of all ages.

This position is for an Independent Contractor to serve the San Gabriel Valley area specifically.

The Occupational Therapist in Home Health is responsible for the assessment and evaluation of patient care needs relating to functionality status, activities of daily living, fine motor coordination, home assessments and adaptive equipment, and other occupational therapy needs as defined by medical conditions. Based on this assessment and evaluation, the Occupational Therapist determines a treatment plan, performs interventions aimed at improving and enhancing the patient's wellbeing, and evaluates the patient’s progress.

An OccupationalTherapist for Home Health must:

• Provide services defined by the state laws governing the practice of Occupational Therapy, in accordance with the plan of care, and in coordination with other members of the health care team
• Lead the patient in the use of therapeutic, creative, and self-care activities to improve functioning and safety
• Observe, record, and report the patient's response to treatment and changes to the patient's condition
• Instruct patients, families, caregivers, and the health care team in the areas of therapy in which they can participate in assisting the patient

Qualifications:

• Ability and enthusiasm in working with all ages
• Occupational Therapistlicense and registration by the state
• Completion of an accredited Occupational Therapistprogram
• Bilingual is a plus!
