This Independent Contractor role involves assessing functional needs, developing a treatment plan, and educating patients and caregivers while coordinating with the health care team.

Qualifications

• Completion of an accredited Occupational Therapist program.
• OT license and registration by the state.

Responsibilities

• Assess and evaluate patient care needs related to function, ADLs, fine motor skills, home assessments and adaptive equipment.
• Develop and implement a treatment plan; perform interventions to improve wellbeing and progress.
• Observe, record and report patient responses to treatment; adjust plan as needed.
• Instruct patients, families, caregivers, and health care team on therapy activities and participation.

Job description

FeldCare Connects, an app-based referral network,is currently seeking a self-motivated Occupational Therapist to deliver premier excellence of care and is enthusiastic about working with adults of all ages.

This position is for an Independent Contractor to serve Galt and the surrounding areas.

The Occupational Therapist in Home Health is responsible for the assessment and evaluation of patient care needs relating to functionality status, activities of daily living, fine motor coordination, home assessments and adaptive equipment, and other occupational therapy needs as defined by medical conditions. Based on this assessment and evaluation, the Occupational Therapist determines a treatment plan, performs interventions aimed at improving and enhancing the patient's wellbeing, and evaluates the patient’s progress.

An OccupationalTherapist for Home Health must:

• Provide services defined by the state laws governing the practice of Occupational Therapy, in accordance with the plan of care, and in coordination with other members of the health care team.
• Lead the patient in the use of therapeutic, creative, and self-care activities to improve functioning and safety.
• Observe, record, and report the patient's response to treatment and changes to the patient's condition.
• Instruct patients, families, caregivers, and the health care team in the areas of therapy in which they can participate in assisting the patient.

Qualifications:

• Ability and enthusiasm in working with all ages
• Occupational Therapistlicense and registration by the state
• Completion of an accredited Occupational Therapistprogram
• Bilingual is a plus!
