Occupational Therapist OT for Home Health

FeldCare Connects, an app-based referral network, is currently seeking a self-motivated Occupational Therapist to deliver premier care and work with adults of all ages.

This position is for an Independent Contractor serving Los Angeles and surrounding areas.

The Occupational Therapist in Home Health is responsible for assessing and evaluating patient care needs related to functionality, activities of daily living, fine motor skills, home assessments, adaptive equipment, and other occupational therapy needs as defined by medical conditions. Based on this, the therapist develops a treatment plan, performs interventions to improve patient wellbeing, and evaluates progress.

Responsibilities include:

• Providing services in accordance with state laws, the plan of care, and in coordination with the healthcare team.
• Leading patients in therapeutic, creative, and self-care activities to enhance functioning and safety.
• Observing, recording, and reporting patient responses and condition changes.
• Instructing patients, families, caregivers, and the healthcare team in therapy participation.

Qualifications:

• Ability and enthusiasm to work with all ages.
• Occupational Therapist license and registration by the state.
• Completion of an accredited Occupational Therapist program.
• Bilingual skills are a plus!
