Job description

FeldCare Connects, an app-based referral network, is currently seeking a self-motivated Vietnamese-speaking Occupational Therapist to deliver excellent care and work with adults of all ages.

This position is for an Independent Contractor serving Rosemead and surrounding areas.

The Occupational Therapist in Home Health is responsible for assessing and evaluating patient care needs related to functional status, activities of daily living, fine motor coordination, home assessments, adaptive equipment, and other occupational therapy needs as defined by medical conditions. Based on this assessment, the therapist determines a treatment plan, performs interventions to improve the patient’s wellbeing, and evaluates progress.

An Occupational Therapist for Home Health must:

1. Provide services according to state laws and in accordance with the plan of care, in coordination with the healthcare team.
2. Lead patients in therapeutic, creative, and self-care activities to improve functioning and safety.
3. Observe, record, and report patient responses and condition changes.
4. Instruct patients, families, caregivers, and healthcare team members on therapy participation.

Qualifications:

1. Ability and enthusiasm for working with all ages.
2. Occupational Therapist license and registration by the state.
3. Completion of an accredited Occupational Therapist program.
4. Bilingual skills are a plus!
