What You'll Be Doing:

• Creating custom therapy plans to help patients thrive at home
• Implementing therapeutic activities that improve daily life
• Teaching patients and family's practical tools for success
• Collaborating with physicians, COTAs, nurses, and other teammates
• Tracking progress and adjusting care plans as needed
• Keeping clear and timely documentation (we like organized heroes!)
