Job summary

FeldCare Connects seeks a self-motivated Occupational Therapist to provide home health services in Chico and surrounding areas. The contractor will assess ADLs, functional status, and equipment needs, then design and implement a plan of care.

The role emphasizes patient safety, progress assessment, and collaboration with families and the health care team. Bilingual skills are a plus, and independence is encouraged.

Qualifications

• Licensed and registered Occupational Therapist in the state.
• Completion of an ACOTE-accredited OT program.
• Bilingual abilities are a plus.

Responsibilities

• Assess and evaluate patient care needs related to functionality, ADLs, fine motor skills, home assessments and adaptive equipment.
• Develop and modify a treatment plan to improve patient wellbeing and progress.
• Perform interventions to improve functioning and safety; document responses to treatment.
• Lead patients in therapeutic activities and educate families and caregivers.
• Observe, record, and report changes in patient condition to the care team.

Job description

FeldCare Connects, an app-based referral network, is currently seeking a self-motivated Occupational Therapist to deliver premier excellence of care and is enthusiastic about working with adults of all ages.

This position is for an Independent Contractor to serve Chico and the surrounding areas.

The Occupational Therapist in Home Health is responsible for the assessment and evaluation of patient care needs relating to functionality status, activities of daily living, fine motor coordination, home assessments and adaptive equipment, and other occupational therapy needs as defined by medical conditions. Based on this assessment and evaluation, the Occupational Therapist determines a treatment plan, performs interventions aimed at improving and enhancing the patient's wellbeing, and evaluates the patient’s progress.

An OccupationalTherapist for Home Health must:

• Provide services defined by the state laws governing the practice of Occupational Therapy, in accordance with the plan of care, and in coordination with other members of the health care team.
• Lead the patient in the use of therapeutic, creative, and self-care activities to improve functioning and safety.
• Observe, record, and report the patient's response to treatment and changes to the patient's condition.
• Instruct patients, families, caregivers, and the health care team in the areas of therapy in which they can participate in assisting the patient.

Qualifications:

• Ability and enthusiasm in working with all ages
• Occupational Therapistlicense and registration by the state
• Completion of an accredited Occupational Therapistprogram
• Bilingual is a plus!
