Job summary

FeldCare Connects is seeking a self-motivated Occupational Therapist to deliver premier in-home care for adults in La Quinta and surrounding areas. This Independent Contractor role focuses on functional assessment, ADLs, and adaptive equipment needs within home health.

The OT will assess, plan, and intervene to improve patient wellbeing, coordinating with the care team to monitor progress. California licensure is required; bilingual skills are a plus and preferred.

Qualifications

• OT license and California registration required.
• Completion of an accredited OT program.
• Ability and enthusiasm to work with adults of all ages.
• Bilingual is a plus.

Responsibilities

• Assess patient needs related to activities of daily living, functional status, and home safety.
• Develop and implement an in-home treatment plan to improve wellbeing and function.
• Observe, record, and report patient progress to the care team; adjust plan as needed.
• Educate patients, families, and caregivers on therapy techniques and safety.

Job description

FeldCare Connects, an app-based referral network, is currently seeking a self-motivated Occupational Therapist to deliver premier excellence of care and is enthusiastic about working with adults of all ages.

This position is for an Independent Contractor to serve La Quinta and the surrounding areas.

The Occupational Therapist in Home Health is responsible for the assessment and evaluation of patient care needs relating to functionality status, activities of daily living, fine motor coordination, home assessments and adaptive equipment, and other occupational therapy needs as defined by medical conditions. Based on this assessment and evaluation, the Occupational Therapist determines a treatment plan, performs interventions aimed at improving and enhancing the patient's wellbeing, and evaluates the patient’s progress.

An OccupationalTherapist for Home Health must:

• Provide services defined by the state laws governing the practice of Occupational Therapy, in accordance with the plan of care, and in coordination with other members of the health care team.
• Lead the patient in the use of therapeutic, creative, and self-care activities to improve functioning and safety.
• Observe, record, and report the patient's response to treatment and changes to the patient's condition.
• Instruct patients, families, caregivers, and the health care team in the areas of therapy in which they can participate in assisting the patient.

Qualifications:

• Ability and enthusiasm in working with all ages
• Occupational Therapistlicense and registration by the state
• Completion of an accredited Occupational Therapistprogram
• Bilingual is a plus!
