Job summary

Evolve Therapy Services seeks a dedicated Occupational Therapist for an in-house Skilled Nursing Facility in East Moline, IL. The role requires planning, directing, and delivering occupational therapy services to residents, in accordance with federal, state, and local standards, under the direction of the Administrator.

The candidate should hold an active OT license in the state and participate in ongoing licensure-related education to maintain certification.

Qualifications

• Must hold an active Occupational Therapy license in the practicing state.
• Engage in ongoing continuing education to maintain licensure and certification.

Responsibilities

• Conduct regular resident screenings to determine best intervention or treatment.
• Evaluate residents to inform and develop treatment plans with goals.
• Communicate evaluation findings, treatment plans, and recommendations to residents, families, and inter-disciplinary team.
• Establish individual resident therapy schedules with team members.
• Provide Occupational Therapy services per treatment plan and ensure proper documentation and billing.
• Perform additional duties as needed to effectively treat patients.
• Oversee COTA as needed and ensure documentation and reimbursement compliance.

Job Position:

Occupational Therapist for an In-house Skilled Nursing Facility

Job Type:

Full Time or PRN

Job Purpose:

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed

Education & Qualifications:

• Degree in Occupational Therapy
• MUST have an Active Occupational Therapy license in practicing state(s)
• Ongoing continuing education and professional development to maintain current licensure and certification
