Job summary

Evolve Therapy Services is seeking a compassionate Occupational Therapist for an in-house Skilled Nursing Facility in Freeport, IL. This PRN role focuses on planning, organizing, and directing occupational therapy services to ensure high-quality resident care.

The OT will screen and evaluate residents, develop treatment plans with achievable goals, and collaborate with an interdisciplinary team. Documentation and billing accuracy are essential, with supervision of COTA as needed.

Qualifications

• Must hold an active Occupational Therapy license in practicing state(s).
• Ongoing continuing education and professional development to maintain licensure.

Responsibilities

• Plan, organize, develop, and direct Occupational Therapy Services in the facility.
• Screen residents to determine best intervention or treatment.
• Evaluate residents and develop treatment plans with goals.
• Communicate evaluation findings and plans to residents, families, and team.
• Establish therapy schedules with team members.
• Provide therapy per plan and ensure proper documentation and billing.
• Oversee COTA as needed and ensure compliant practices.

Job Position: Occupational Therapist for an In-house Skilled Nursing Facility

Job Type: PRN

Job Purpose: To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient

• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals

• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members

• Establish individual resident therapy schedules in cooperation with other team members

• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services

• Perform all other duties as assigned and as required to effectively treat patients

• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed

Education & Qualifications

• Degree in Occupational Therapy

• MUST have an Active Occupational Therapy license in practicing state(s)

• Ongoing continuing education and professional development to maintain current licensure and certification
