Job summary

Evolve Therapy Services in New Haven, CT is seeking a dedicated Occupational Therapist for an in-house Skilled Nursing Facility. The role focuses on delivering high-quality OT services to residents and collaborating with an interdisciplinary team.

You will evaluate residents, develop treatment plans, document progress, and mentor COTA staff. Full-time or PRN positions available; a valid OT license in Connecticut is required and ongoing continuing education is encouraged.

Qualifications

• OT degree required.
• Active OT license in practicing state(s).
• Ongoing continuing education to maintain licensure.

Responsibilities

• Screen residents to determine best intervention or treatment.
• Evaluate residents to inform treatment plans and goals.
• Communicate evaluation findings and plans to residents, families, and team.
• Establish therapy schedules with team
• Deliver therapy per plan and document/bill services
• Perform other duties as assigned to treat patients
• Oversee COTA and ensure documentation and reimbursement practices are followed

Job Position: Occupational Therapist for an In-house Skilled Nursing Facility

Job Type: Full Time or PRN

Job Purpose: To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed

Education & Qualifications:

• Degree in Occupational Therapy
• MUST have an Active Occupational Therapy license in practicing state(s)
• Ongoing continuing education and professional development to maintain current licensure and certification
