Job summary

Embassy of Hearthside is seeking a compassionate full-time Occupational Therapist (OT) to join our in-house rehabilitation team in State College, PA. This role focuses on improving the independence, safety, and quality of life of residents through personalized therapy.

You'll evaluate functional abilities, develop treatment plans, and collaborate with a dedicated care team. New graduates are welcome to apply.

Qualifications

• Active Occupational Therapy license in PA, in good standing.
• Skilled nursing or long-term care experience preferred.
• Strong clinical, communication, and organizational skills.

Responsibilities

• Evaluate residents’ functional abilities and their impact on daily activities.
• Develop and implement individualized occupational therapy treatment plans.
• Provide skilled therapy services focused on independence in daily living.
• Recommend adaptive equipment and home modifications as appropriate.
• Monitor resident progress and adjust OT treatment plans as needed.
• Educate residents, families, caregivers, and clinical staff.
• Collaborate with the interdisciplinary care team.
• Maintain accurate, timely, and compliant documentation.
• Ensure services meet all professional, legal, and regulatory standards.
