Job summary

Wabasso Rehabilitation and Wellness Center in Minnesota is seeking an Occupational Therapist (OT) to evaluate residents, plan care, and deliver therapeutic services aimed at promoting independence and safety.

You will work with a multidisciplinary team, document progress, educate residents and families, and ensure compliance with state licensure and professional standards. Experience in long-term care is preferred.

Qualifications

  • Active Minnesota OT license required.
  • Graduate of an accredited OT program.
  • Strong knowledge of OT principles and equipment.
  • Excellent communication and organizational skills.

Responsibilities

  • Evaluate residents and develop individualized treatment plans.
  • Provide OT treatments, ADL training, and equipment recommendations.
  • Document evaluations, progress notes, and discharge summaries.
  • Collaborate with physicians, nurses and therapy staff for coordinated care.
  • Educate residents, families and staff on interventions and safety.
  • Monitor progress and adjust plans to meet goals.
  • Maintain compliance with Minnesota OT Practice requirements.

Occupational Therapist (OT)
Position Summary:

The Occupational Therapist (OT) is responsible for evaluating, planning, and providing occupational therapy services to individuals in accordance with federal and state regulations, professional standards of practice, and licensure requirements. The OT works with residents, families, and interdisciplinary team members to promote independence, safety, and improved quality of life.

Key Responsibilities:
  • Evaluate residents’ functional abilities and develop individualized treatment plans

  • Provide direct occupational therapy treatments, including therapeutic exercises, ADL (Activities of Daily Living) training, and adaptive equipment recommendations

  • Document evaluations, progress notes, and discharge summaries in compliance with state and federal guidelines

  • Collaborate with physicians, nurses, and other therapy staff to ensure coordinated, resident-centered care

  • Educate residents, families, and staff on therapy interventions, adaptive techniques, and safe practices

  • Monitor resident progress and adjust treatment plans as necessary to achieve goals

  • Maintain professional and ethical standards, including adherence to Minnesota Board of Occupational Therapy Practice requirements

Qualifications:
  • Current and active Occupational Therapist (OT) license in Minnesota

  • Graduate of an accredited occupational therapy program

  • Strong knowledge of occupational therapy principles, techniques, and equipment

  • Excellent communication, organizational, and interpersonal skills

  • Ability to work collaboratively within a multidisciplinary team

  • Previous experience in long-term care or rehabilitation setting preferred
