Job summary

The Grand Rehabilitation and Nursing at Barnwell is seeking a licensed Occupational Therapist for a full-time role in upstate New York, 25 minutes from Albany. The position features a day shift in a skilled nursing/rehabilitation setting.

The OT will evaluate, develop, and implement individualized treatment plans to improve residents’ functional independence, with a focus on patient-centered care, interdisciplinary collaboration, and work-life balance.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current New York State OT license required.
• Experience in skilled nursing or long-term care preferred (not required).
• Strong clinical judgment, communication, and documentation skills.
• Ability to work independently and as part of a collaborative rehab team.

Responsibilities

• Evaluate residents and develop individualized occupational therapy treatment plans
• Implement treatment plans focused on improving functional independence and daily living skills
• Assess and document resident progress through daily notes, progress reports, and re-certifications
• Maintain accurate and compliant clinical documentation
• Ensure confidentiality of resident information at all times
• Supervise Occupational Therapy Assistants (COTAs) as assigned
• Participate in care plan meetings and interdisciplinary collaboration
• Assist with departmental projects, reporting, and data tracking as directed by the Rehab Director
