Job summary

Achieve Rehab & Nursing Facility – Liberty, NY is seeking a compassionate Full-Time Occupational Therapist (OT) to join our in-house rehabilitation team in Liberty. You will evaluate residents and develop individualized OT plans to help them regain independence and improve daily function.

Responsibilities include addressing ADLs, upper extremity function, cognition, adaptive equipment needs, and collaborating with physicians and the interdisciplinary team.

Qualifications

• Licensed or eligible for New York OT license.
• Graduate of an accredited Occupational Therapy program.
• New graduates are encouraged to apply.
• SNF/LTC experience preferred but not required.
• Experience with Net Health and PCC preferred.

Responsibilities

• Evaluate residents and develop individualized OT treatment plans.
• Provide skilled OT services to short-term rehab and long-term care residents.
• Address ADLs, upper extremity function, cognition, adaptive equipment, and mobility.
• Monitor progress and modify treatment plans based on outcomes.
• Educate residents, families and caregivers on interventions and safety techniques.
• Document care accurately using Net Health and PCC.
• Collaborate with physicians and the interdisciplinary team to maximize outcomes.
• Maintain compliance with all regulations.

• Complete accurate and timely documentation using Net Health and PointClickCare (PCC).
• Collaborate with physicians, nursing staff, and the interdisciplinary care team to maximize resident outcomes.
• Maintain compliance with all federal, state, and facility regulations.

Qualifications

• Current New York Occupational Therapist (OT) license or eligibility for licensure.
• Graduate of an accredited Occupational Therapy program.
• New graduates are encouraged to apply.
• Skilled Nursing Facility (SNF) or Long-Term Care experience is preferred but not required.
• Experience with Net Health and PointClickCare (PCC) is preferred.
• Strong communication, clinical reasoning, and organizational skills.
• Passion for providing compassionate, resident-centered care.
