Job summary

Signature HealthCARE is seeking an Occupational Therapist to join our North Hardin Rehab & Wellness Center in Radcliff, KY. You will deliver occupational therapy to residents, contributing to recovery and daily living activities, while collaborating with physicians, nurses and therapy colleagues.

You will design and implement individualized treatment plans, monitor progress, and document outcomes to support quality, person‑centered care across the Signature HealthCARE network.

Qualifications

• Graduate of an accredited occupational therapy program.
• Must hold NBCOT certification or be eligible for certification.
• Valid occupational therapy license in the practice state.

Responsibilities

• Provide occupational therapy services to residents and patients to promote recovery and independence.
• Collaborate with the interdisciplinary care team to plan and execute treatment programs.
• Develop, implement and document individualized OT treatment plans and progress notes.

How You Will Make a Difference

Under Signature Rehab’s integrated care model rehabilitative therapist work closely with other clinical caregivers to ensure residents and patients make a full recovery and a timely return to the activities they enjoy.

What You Need To Make a Difference

• Graduate of an accredited program of occupational therapy, culminating in a minimum of a bachelor‑of‑science degree.
• Current certificate with the National Board for Certification in occupational therapy or eligible for certificate as required by state guidelines.
• Valid occupational therapy license in the state where services are rendered.
